If you wish to obtain your IRS Form 1095-B for 2021, you may submit this request to the Fund Office in the following ways:
Email: ACA1095B@nyccbf.org
Paper mail: ATTN- NYCDCC Welfare Fund, 395 Hudson Street, 9th Floor, New York, New York, 10014.
Please note that the Fund Office is required to furnish a Form 1095-B to an individual upon request within 30 days of the date the request is received.
*Due to a change in federal law, you are no longer required to submit an IRS Form 1095-B (which documents health insurance coverage) when filing your federal tax returns. As a result, the NYCDCC Welfare Fund will no longer automatically mail the Form 1095-B to all participants. However, the Welfare Fund will continue to mail the Form 1095-B to participants living in states, such as New Jersey, California, and Rhode Island, where there is a requirement that residents include the form with their local tax returns.
