Attention Members:
Your IRS Form 1095-B is available upon request.
Due to a change in federal law, you are no longer required to submit an IRS Form 1095-B (which documents health insurance coverage) when filing your federal tax returns. As a result, the New York City District Council of Carpenters Welfare Fund will no longer automatically mail the Form 1095-B to members unless the member lives in California, New Jersey, Rhode Island, or the District of Columbia due to requirements that may require residents in those jurisdictions to include the form with their local tax returns.
If you wish to obtain your IRS Form 1095-B, you may do so by writing to the New York City District Council of Carpenters Welfare Fund, 395 Hudson Street, New York, NY 10014, Attention: 1095-B Request, or by emailing ACA1095B@nyccbf.org.
