Reciprocal Authorization Form – Request form for Welfare, Pension and/or Annuity contributions

Benefit Opt In Form – An Outside member’s request for all or a portion of their Benefits to remain with the NYCDCC Benefit Funds.

Welfare Benefit Opt In Form – An Outside member’s request for Welfare Benefits portion to remain with the NYCDCC Benefit Funds and request to use the NYCDCC Welfare Plan.

Authorization to Rescind Reciprocal Form – Cancellation of previously signed Reciprocal form.
