Notice for NYCDCC Retirees and Surviving Spouses Currently Receiving a Pension

NYCDCC Retirees and Surviving Spouses: You may have already received, or will soon be receiving a mailing from the NYCDCC Benefit Funds requiring you to complete and sign an affidavit attesting to the fact that you are the rightful individual receiving your retirement benefits. This process is occasionally undertaken by the Pension Plan to prevent fraud by confirming that our participants are alive and therefore eligible to continue receiving benefits. Please note that completing and returning this form is MANDATORY. Any retiree or surviving spouse who does not return this form in a timely fashion will have their pension benefits suspended as of October 1, 2025.
