Requests for Copies of Form 1095-B

The Fund Office would like to remind you that you may request a copy of your 2020 Form 1095-B at any time.

You may submit these requests to the Fund Office in the following ways:

Paper mail: ATTN- NYCDCC Welfare Fund, 395 Hudson Street, 9th Floor, New York, New York, 10014.

Please note that the Fund Office is required to furnish a Form 1095-B to an individual upon request within 30 days of the date the request is received.
